Back to topThe Form Builder extension allows you to create numerous forms through the admin panel easily. The website owners / Digital marketing managers want to create a form for various purposes based on their different requirements, it is a good choice of this extension. It allows you to create various fields from the admin panel. Also, there is an option for the group the fields as field set as it would help you define the section in an organized way.
There are various settings to apply the forms will be created. It has a feature to notify the admin as well as a customer through email when any forms have been submitted by the customer At the front end of the website. Additionally, it has a feature to allow you to create a product inquiry form in the product tab section. This would be more helpful to set inquiries per product.

Review all the forms data in individual form grid. Merchant can view data against the created field of the form.
Render:
Form builder allows creating an individual page using the URL key. And also admin can create a widget for form builder to display anywhere on the website. Admin can create a form builder widget with a specific form.
Publish:
Admin can create a URL key for displaying form through form configuration and also it allows the admin to configure the breadcrumbs through store configuration.

Email Notification:
- Admin can receive e-mail notification when a form is submitted by a customer based on each form settings/configuration.
- Customers can receive e-mail notification when a form is submitted based on settings.
- Admin can customize e-mail templates through Marketing->Email Templates.
Security:
Prevent automated form submission, used Magento ReCaptcha to ensure form submitted by humans or Admin can configure Google Recaptcha in the form. Admin needs to configure the captcha to allow the form builder to use it. It can be access through Store->Configuration->Customers->Customer Configuration->Captcha->Forms.
Submission:
- Admin can configure custom messages for success and failure when a form is submitted.
- Admin can configure the URL to redirect after form submission.

- Enable/disable the Form Builder Extension.
- Add custom URL for each and every form for better SEO practice.
- Add content using the WYSIWYG editor to show the form's top and bottom of each form.
- Add fieldset to group the fields for each form.
- Add the custom class for field set to apply branding as per the theme
- You can also add a Product Inquiry form in product pages which will be displayed in the product tabs section. You can manage the tab title and corresponding form from the admin configuration.
- Admin can create a form builder widget to display it in any place of the website.
- Each form of values will be displayed on the individual grid list. It will create the field columns dynamically for all fields from the individual form.
- Admin and customer email templates can be managed in admin configuration. And email templates can be customized in the Magento email templates section.
- You can add Magento captcha for every form with individual configurations.
- Enable/disable breadcrumbs in the form view page.
- You can change the date field format from the admin configuration.
- Form submit redirect URL can be managed from the individual form. Enter redirect URL or leave blank to redirect on the same form page.
- Admin can delete submitted data from form data grid using mass delete feature.
- Admin can export required submitted form data from the mass action feature.

Answer:- Yes, you can add. And state values will be loaded when a country selected.
